Student Campus Access Request Form (LS OSS Memo)

We know you are excited to enter the campus for the first time in years, but please be reminded that our priority is still to ensure the health and safety of all on campus, so we are concerned foremost about limiting the number of students entering campus to manage the risk of infection brought about by CoVid-19 and other infectious diseases.

 

Please note that only necessary and authorized activities will be allowed on campus during the pilot 2nd semester period. These include, aside from attending onsite classes, doing essential research in the library which entails the use of physical documents, doing F2F counseling or consultation, or purchasing items at the LS Bookstore.

 

In order to facilitate entry into campus, we have come up with the LS Student Campus Access Request (CAR) Form. This Google Form ensures the capture of necessary information needed for one to enter LS premises while following our health guidelines. This request form is primarily for students who are not authorized to enter campus for regular face-to-face activities and who do not have access to the Blue Pass entry system. But if you are a regular student who is not authorized to get a Blue Pass on the day you need to enter campus, you may also use this CAR Form.

 

Providing necessary information, like your full vaccination status (2 doses), health insurance information and health declaration on the day you are allowed to enter are required for this process.

You will also need to set your appointments with the office, or the Rizal Library facility you are choosing to visit using the information on the CAR Form. Approval is given by the Campus Safety and Mobility Office (CSMO) and one should not proceed to campus without this confirmation email.

 

If your request is denied for good reason, you will receive an email from the office you have set an appointment with.
