Updating of Medical Records First Semester SY 2024-2025

Dear Ateneo College students,

As we approach the end of the semester, I would like to remind you to update your electronic medical records at the BluePHR system. It is important to do this after the release of grades on 26 July 2024, along with updating your AISIS records before the coming enrollment.

Updating your health record is required every semester prior to enrollment, and failure to do so will result in a hold on your AISIS account access which may interfere with enrollment procedures. Please ensure that you enter any new health data if relevant, such as recent vaccinations, medical conditions, immunizations, or newly prescribed medications through the link @ https://bluepass.ateneo.edu. Once you have updated your data, click "submit" at the end of the health record. Note that even if you have no updates, you still need to click submit to signify that you have reviewed your data.

If you have any current medical or psychological conditions, a recent certificate will be required for each semester. Please note that failure to submit recent certification of a current condition will be taken as an indication that no assistance will be needed for the said condition for the semester.

For those who have not yet complied with the requirements for submission of Chest X-ray findings and completed immunization in their online medical record, please upload through the link @ https://bluepass.ateneo.edu.
